• I affirm that I have read the entire parents guide, and my children and I have read the PARENT & STUDENT RESPONSIBILITES together.
• I agree to abide by the rules and expectations as presently enforced or as may be amended.
• I understand that if anyone in my family violates the rules, it will jeopardize our participation in the HOPE Co-op.
• I agree to be responsible for the behavior of my child(ren).
• I understand that I will be required to fulfill the co-op job(s) that will be assigned to me.
• I agree to pay for any item that my child(ren) damages at the facility in which we meet.
• In the case of an incident that results in physical and/or psychological injury to my child(ren) or myself, I release: HOPE Co-op and its agents, HOPE Co-op Advisory Board members, HOPE registrants, and the facility in which we meet, from any and all liability and all claims against them, individually or collectively, for any and all damages or medical care necessary.

Hope Home School Co-op

1. Complete one registration box for each child you are registering,includingpreschoolers and babies. Make extra copies if needed.

2. All children attending co-op must be registered for every hour they are on campus.

3. Please indicate your 1^st and 2^nd choice for each period. We will do our best to give each child their first choice.

4. Do not register for a period that is difficult to be on time for! Arriving late is disruptive to the entire class. Please read Late Policy in Parents Guide carefully. Students that are continually late may be asked to drop that class.

5. We cannot guarantee you will get the classes you request!!!

All parents must serve in a volunteer capacity 3 out of the 4 hours their children are enrolled in Co-op classes. Please check age groups you are interested in working with:

There are many jobs that will need to be performed such as teacher assistants, classroom monitors, hall monitors, and facility caretakers. If you have a specific class you would like to help in, please make a note of it below. We will try to accommodate you.
